Transcript Request-Form



How to submit an online Transcript Request.


Email Subject must be: Grossmont Transcript Request

Attachment must be sent to: (do not use the default email)

  1. Click on the link below (you will need Adobe Reader ver. 8 or higher)  Get Adobe Reader
  2. Fill the PDF request form
  3. Digitally sign the request (if have not created a digital signature before, see instruction)
  4. Email your request
    • Save the Form to your local computer
    • Login to you internet email
    • Attach the PDF form to the email
    • Enter or change the subject to  Grossmont Transcript Request
    • Send the email with the attachment to (this email is not monitored, for inquiry see contact below)

If you do not receive an email reply acknowledging that we have received your Transcript request within one working day, check your junk/trash folder otherwise most likely your send is not successful, please send again. (Please note, we are closed weekends and holidays)

Email Subject must be: Grossmont Transcript Request (without Grossmont Transcript Request in the subject (cut and paste if you have too) will result in delay, no confirmation, and possibly the lost of your request.

If you are unable to submit the form online, then print and fax (619.644.7933) or mail it.
